Pharmacological properties
combined preparation for external use. Flumethasone is a fluorinated cortisone derivative with a pronounced anti-inflammatory, antiallergic and antiexedative action. The drug eliminates or reduces the severity of the inflammatory process in the skin. The salicylic acid contained in the preparation has a keratolytic and emollient effect, promoting the penetration of flumethasone into the skin, as well as a weak antiseptic effect. Flumethasone pivalate penetrates well into the skin through the horny layer of the epidermis. After systemic resorption, it is metabolized in the liver, excreted in the urine in the form of glucuronic conjugates and unchanged. Salicylic acid is not practically absorbed when applied topically, however it facilitates the penetration of flumethasone pivalate through the keratinized epidermis
Indications for use
inflammation of the sebaceous glands, atopic, contact, allergic, seborrheic dermatitis, lichen planus, eczema with lichen, polymorphic erythema, discoid lupus erythematosus, prurigo, psoriasis with prolonged course
Dosing and Administration
for topical application. Ointment is applied a thin layer on the affected area of ​​the skin at the beginning 2-3 times a day, and after the weakening of acute phenomena 1-2 times a day. It is possible to apply the drug under the occlusive dressing. At the onset of clinical recovery, treatment is continued for another 3-4 days. Do not continue therapy for more than 3 weeks. Do not apply to the skin for more than 1 week. For a week it is recommended to consume no more than 1 tube of cream. With significant lichenization, the use of an occlusive dressing is indicated, which should be changed every 24-48 hours.
Side effects
steroid acne, purpura, impaired regeneration of the epidermis, atrophy of subcutaneous fat, dry skin, hypertrichosis or alopecia, depigmentation or hyperpigmentation of the skin, atrophy and skin integrity disorders, telangiectasia, perioral dermatitis, folliculitis, secondary infections. In isolated cases, hives, maculopapular rashes, or exacerbation of existing skin changes may occur. Applying ointment to the occlusive dressing due to increased resorption can have a systemic effect, which manifests itself in the form of peripheral edema, hypertension, a decrease in resistance to infections, extremely rarely - the development of cataracts and glaucoma.
Contraindications
increased sensitivity to GCS, salicylic acid or other components of the drug, pustular skin diseases, bacterial, fungal and viral (herpes, shingles, chickenpox) skin infections, skin tuberculosis, skin tumors, ordinary and rosacea, skin diseases complicated by a secondary bacterial infection, children under 5 years, skin integrity disorders, burns, I trimester of pregnancy.

Overdose
with prolonged use of the drug on large areas of the skin, it is possible to develop side effects that are characteristic of systemic use of GCS (peripheral edema, hypertension, hyperglycemia, reduction of resistance to infections), and in severe cases, the development of Cushing's syndrome. Therapy is symptomatic. However, given the low degree of systemic resorption of the drug with external application, these symptoms occur very rarely, and only with its continued use (more than 2 weeks).
Application features
The use of Lorinden A ointment is indicated for dry inflammatory conditions of the skin, especially allergic etiology, not complicated by a secondary bacterial infection, which are accompanied by lichenization and persistent itching. Do not use the drug for more than 2 weeks. With prolonged use in large areas of the skin, the incidence of side effects increases. Avoid application of the drug to the eyelids or the skin around the eyes in patients with glaucoma, as well as in patients with cataracts, given the possibility of worsening of the course of the disease. On the skin of the face, inguinal and axillary areas are used only in cases of extreme necessity, since it is possible to increase the degree of absorption and development of side effects (telangiectasia) even after a short application. It should be used with caution in the presence of atrophy of subcutaneous fat, especially in the elderly. When treating chronic diseases, for example, psoriasis or chronic eczema, you can not abruptly cancel the drug. Children and adolescents prescribed ointment when absolutely necessary and only in small areas of the skin. Do not apply the product on the face. It is not recommended to prescribe the drug during pregnancy, especially long, and on large areas of the skin. During the period of breastfeeding, short-term use of the drug in small areas of the skin is allowed. Do not apply the product on the skin of the mammary glands. During the period of treatment it is not recommended to vaccinate against smallpox, as well as other types of immunization.
